Community Health Workers are trusted connectors. They listen to a person’s needs, explain available options in plain language, and help connect people with programs related to health, food, housing, transportation, education, employment, and family well-being.
Our work is not about diagnosing or replacing medical professionals. It is about helping people navigate resources, complete appropriate applications, understand next steps, and feel supported throughout the process.
A position of trust
Professional conduct is not optional.
CHWs may work with people during vulnerable moments. Every interaction must protect the dignity, privacy, and confidence of the person seeking help.
Act with integrity
Never promise approvals, benefits, outcomes, or services that cannot be guaranteed.
Communicate respectfully
Listen without judgment and treat every person with patience and professionalism.
Follow the process
Complete required training, document work accurately, and follow program guidance.
